REGISTERING FOR UNEMPLOYMENT INSURANCE

New employers in the state of Louisiana can register with the Louisiana Workforce Commission by visiting the website . You will need click the box at the bottom of the page after reading the checklist and then click Begin Application.

Once registered, you will receive an 
Unemployment Account Number (UI) (called “Unemployment Insurance Contribution Rate”) and Unemployment Rate (%) which you will need to enter into Namely Payroll under Company > Tax screen (UI tax).

If you’ve previously run payroll in the state, your 
EAN, sometimes called your State Identification Number (SID)SUTA ID Number or Unemployment Account Number can be found on the Liability Letter sent to you by the Louisiana Workforce Commission or on your Quarterly Report of Wages Paid (Form LDOL-ES4).

REGISTERING FOR INCOME TAX WITHHOLDING  

New employers in the state of Louisiana can register with the Louisiana DOR by visiting the website. Click Start New Application and follow the prompts.

TIP:

Future-date registration is possible for this state's income tax withholding! 

Once registered, you will receive a Withholding Tax Account Number (WH) which you will need to enter into Namely Payroll under Company > Tax (WH tax). 

If you've run payroll in Louisiana previously, you can find your 
DOR account number on your Employer's Return of Income Tax Withheld (L-1) that was mailed to you by the Louisiana DOR.

POWER OF ATTORNEY FORMS

Please note, Namely does not require a Power of Attorney for this state at this time. It is not required to file or remit payments on your behalf. If one is needed to resolve a tax matter, the tax notice team will request one from you at that time.

If a Power of Attorney is requested from you, you should email completed copies of the forms 
namelyPOA@Namely.com with the CID in the subject line for processing. We will update our system and maintain a copy of each form for record-keeping purposes

STATE INCOME WITHHOLDING FORMS

Namely uploads Federal and State W-4 documents to your HRIS site for use during onboarding, or if you need to send W-4s to employees who need to make withholding adjustments.

TIP:

The most up to date W-4 documents can be found in the Namely section of the Document dropdown menu in Onboarding and Manage eSignature


Form Name: R-1300 (L-4)

Accepts Federal W-4: No

Comments: Each employee must provide the employer with a completed Form L-4, Employee's Withholding Exemption Certificate, or Form L-4E, Exemption from Withholding Certificate. These certificates indicate the number of personal exemptions and credits for dependents claimed.
